Add Media button only highlights... Doesn't work

I am having an issue … in Chrome, it will work only 1/2 the time, but it will not work on IE, Firefox or Opera.
See attached screenshots… the chrome instance is the only time it works … and only when it looks that way. If you add something, you cannot add a second because it reverts to the second look.

Hi,
This looks like some plugin or theme conflict. Could you try deactivating the other plugins and trying it on the default theme.
